Indiana Last Will and Testament Sample

Testator: Michael Johnson

Declaration of Intent:

I, Michael Johnson, being of sound mind, declare this to be my Last Will and Testament. I revoke all previous wills and codicils.

Appointment of Executor:

I hereby nominate and appoint Sarah Williams of Indianapolis, Indiana, as the Executor of my estate. If she is unable or unwilling to serve, I appoint Mark Davis as successor Executor.

Distribution of Assets:

I give, devise, and bequeath my estate as follows:

• To my spouse, Lisa Johnson, my primary residence located at 789 Maple Street, Indianapolis, IN 46204.
• To my children, Emily Johnson and Ryan Johnson, all remaining personal property and residual estate, equally divided.

Guardian for Minor Children:
